GeRas – Ensuring geriatric rehabilitation success in the long term
In addition to treating motor deficits in older people, geriatric rehabilitation primarily aims to enable patients to return to their home environment. However, there are currently no structured programs available in standard care to accompany this part of the care of older people with multimorbidity.
The aim of the project is to ensure the success of rehabilitation for geriatric patients after hospitalization. To this end, a coordinated treatment program with and without telemedical support is to be tested to ensure continuity of care between inpatient rehabilitation and outpatient follow-up care upon discharge to the home environment. Core elements are home visits by service providers from the rehabilitation clinic and care managers from the health insurance company, telephone calls and interdisciplinary case discussions with decreasing frequency. Telemedical services such as video-assisted training and video calls are also to be tested.
As part of a multicentre randomized controlled trial with two intervention groups and a control group, a total of 270 geriatric test subjects will be observed over a period of twelve weeks after discharge from inpatient care. In addition to recording changes in functional disorders of the lower extremities, participation in everyday life is also assessed twelve weeks after discharge. As part of this project, the Institute for Health Economics and Health Services Research at the UKE/HCHE is conducting a health economic evaluation. The consortium leader is the AGAPLESION BETHANIEN KRANKENHAUS HEIDELBERG. The project will be funded for three and a half years with a total of around 4 million euros.
If successful, the aftercare service should be transferred to standard care and help to ensure that the transition to the home environment is coordinated "from a single source" and that rehabilitation successes are secured. It is also expected to have a positive impact on the motor skills of geriatric patients, their independence and participation in everyday life.
