Health Coaching for Chronic Diseases

Chronic diseases such as Diabetes mellitus, cardiac diseases and cancer are among the leading causes of death and health impairments. The impairments are at least partially preventable by improvement of patients’ self-management.

In a joint project the KKH Kaufmännische Krankenkasse assigned the implementation of an individual, cross-sectoral support of patients with higher treatment risks by offering telephone coachings. The long-term counseling for chronically ill and acutely at-risk KKH policyholders took place in four federal states with 19 coaches and a nutritionist.

Researchers at HCHE analyzed effectiveness and efficiency of the pilot project and thus provided the decision-makers with health economic related data analyses.
